Benefits of Home Care – Each patient is different

Our services are available in Colquitt and Worth counties, seven days a week, 24 hours a day to patients in their homes.  Individualized treatment plans are developed with the guidance of the patient’s physician.

Services

Skilled Nursing  Registered nurses provide pediatric or adult nursing care in the home 24 hours a day, 7 days a week.  Nurses provide services for adult and pediatric patients; including wound care, IV therapy and many more.

Personal Aide Services  Certified home health aides provide assistance in daily living activities.

Rehabilitation Services  Licensed therapists provide physical, speech and occupational therapies needed to help the patient gain greater functional independence.

Medical Social Services  Our medical social workers are available to assist patients with personal, family and financial problems related to home healthcare needs.

Coverage for care With a physician referral, Colquitt Regional Home Care accepts Medicare, Medicaid, private pay, private insurance and worker's compensation.

Hospice

Surrounded by Love and Comfort

Colquitt Regional Hospice allows terminally ill patients to spend their last days with dignity in their own homes, surrounded by their loved ones. Our team of trained professionals provides medical care and emotional, spiritual and physical support to patients in the final stages of a terminal illness – and to their families, as well. We emphasize the quality of life, rather that the length of life.

Round the Clock Resources

Colquitt Regional Hospice care is available to patients, seeking palliative treatment, with a life expectancy of six months or less. Our services and support include:

  • Nursing services in the home, assisted living center or nursing home facility
  • Registered Nurse on call for emergencies 24 hours a day, seven days a week
  • Medication to manage symptoms and relieve pain
  • Physical, occupational and speech-language therapy
  • Medical supplies and appliances
  • Home health aide and homemaker services
  • Short-term inpatient care, including respite care
  • Spiritual, dietary and other counseling
  • Social services
  • Volunteer services for companionship and support
  • Bereavement counseling
  • Interdisciplinary team of physicians, nurses, social workers, counselors, clergy, aides and volunteers

 

Private Duty Services

We offer the following services:

Personal Care Services.  Our trained and experienced staff provides one-on-one assistance with the client's activities of daily living (ADLs).  We are available two or more hours per day, seven days a week.  This service enables individuals who live alone to have the assistance they need.  We can assist with:

  • Personal Care
  • Light Housekeeping
  • Medical Preparation
  • Transportation
  • Errands

Home Companionship:  This service is designed for patients who need companionship or homemaking assistance to remain in their own home.  Short-term or long-term arrangements are available.  For clients who live alone or whose companions are unable to assist at home, this service is designed to fill this need.  While in your home our capable staff can assist with all activities of daily living.  Most importantly, we are there to be with the client.  We provide conversation and companionship for those who need daily interaction with others in order to stay active.  In addition, having a companion in the home provides for the client’s safety.

Community Care Services Program (CCSP): Colquitt Regional Private Duty Services is a CCSP provider of this Medicaid waivered program.  Qualified participants may be eligible to receive skilled nursing care, personal care/housekeeping services, respite service, and/or the emergency response system in the home.  Call our office for more information on eligibility.

Emergency Response System (ERS): Our in-home monitoring device, designed for those who live alone and give them the security to continue living independently at home.  Anytime, day or night, when you need help, simply press the personal “help” button.  By pressing the button, the in-home telephone unit is activated and automatically dials directly into Colquitt Regional Medical Center. Our supervisors respond to the alarm and send assistance.

Coverage for care: Colquitt Regional Private Duty Services accepts private pay, private insurance, long-term care insurance, worker's compensation and VA.  We are able to verify insurance benefits and coverage free of charge.  With all the payer sources accepted, arrangements for private duty care are easy as well as convenient.
