Our Mission

The mission of the Hospice of Southwest Georgia is to provide comprehensive care for patients with limiting conditions, within the familiar surroundings of one's own home or home-like setting. 

Hospice offers support services to the family as well as the patient. Education and emotional support for the family members during the illness and bereavement support afterwards are an important part of the scope of hospice care.

Services

Hospice offers a variety of coordinated services provided by skilled professionals and volunteers trained in end-of-life care.

Services are customized to meet the needs of each patient. Under the direction of the physician and the patient’s own wishes, a comprehensive plan of care is developed. This may include any of the following services:

  • Skilled Nursing
  • Hospice Aide
  • Bereavement Counseling
  • Medical Social Work
  • Spiritual Counseling
  • Volunteer Support

Admissions and Payment

Patients under the care of a physician and diagnosed with a life limiting illness are eligible for Hospice care. A primary caregiver must also be available to assist the patient with day-to-day basic needs when self-care is no longer possible.

No one is refused Hospice services on the basis of race, religion, age, sex, ethnic background, handicap condition or financial resources. Anyone may refer a patient to Hospice.  A physician order is required to admit a patient.

Hospice offers services based upon need, rather than ability to pay. Payment for services is accepted from commercial insurance companies, Medicare, Medicaid and private pay.
